Do you need a Family Physician?

We are currently accepting names to our wait list.

Please send an email to info@kemptvillehwc.ca and provide the following information:

Name, address, contact number and email.

Do you currently have a Family Physician?

If yes, please provide the name and address of the Physician.

Please also provide the reason for wishing to discontinue services with your current provider.

Please include information for each family member to be added to the WAITLIST.

  • Due to the extensive volume of people in need of a Family physician, we will be unable to provide a status update on where your name is on the waitlist.
  • Please be assured that if you have provided all of the information requested on the form, we will contact you once a vacancy in one of the Physician practices becomes available.
  • The email provided is designated specifically to adding your name onto our Waitlist ONLY.  All other general enquires must be made by phone through our front reception.

  • We recognize that there is a Physician shortage, both within our community, and provincially.  We are actively working collaboratively with our other community partners to look for solutions to bring additional health care providers to North Grenville.
Although adding your name to our wait list does not guarantee you a space in our clinic, collection of waitlist data allows us to provide statistics to continue to support the stance that additional physicians are needed within our community.
  • We encourage you to visit our website for the latest updates.

* Please note* Personal information is not shared.

Information submitted to the waitlist remains private, secure and confidential.

NOTICE TO ALL CLINIC PATIENTS

  •  If you are experiencing any upper respiratory symptoms, change in smell/taste, and/or fever, please notify us ahead of time by phone. We will triage you over the phone and determine if it is necessary for you to come into the clinic, and if so, we can prepare our staff ahead of time with the necessary Personal Protective Equipment.  You may be asked to take a rapid home Covid test prior to booking or coming in for appointment.
  • As we respond to the increased number of upper respiratory illnesses, our After Hours appointments will be prioritized to these patients.
  • Remember-healthy staff/physicians means better access to care for yourself and your fellow patients!
  • We ask that all interactions with our Staff and Physicians remain courteous and respectful.

WE ARE A TEACHING CLINIC

Yes! Once you have delivered, please call our office to schedule your baby's 1- to 2-week newborn checkup. If your physician is unavailable, our team will arrange for your newborn to be seen by the attending physician. All follow-up appointments will then be scheduled with your regular physician.

If you have a midwife, we still ask that you book an initial visit with us. This will allow us to ensure your child is rostered and scheduled for regular well-baby checkups once your care with the midwife is complete.

Physician Assistants (PA) are highly skilled health care professionals educated in the medical model who practice medicine.

PAs graduate from a Baccalaureate or Master’s Degree program that may be affiliated with a medical school. PAs practice medicine under the supervision of a licensed physician within a patient-centred health care team.

PAs can work in any clinic setting to extend physician services, complement existing services and aid in improving patient access to health care.

  • PAs conduct histories, physical examinations, order and interpret investigations, perform diagnostic and therapeutic interventions, diagnose and treat illnesses, and educate patients on treatment options and counsel on preventative health
  • PAs assist in surgery, prescribe medications, and perform procedures that fall within their scope of training and experience, as long as it falls within the scope of their supervising physician
  • A PA’s practice mirrors that of their supervising physician
