For your physician to be able to bill the Ministry of Health, a valid Ontario health card is needed. Our team will also confirm your address, phone ,e-mail and pharmacy at time of your appointment. This is important to ensure that all information is updated for referrals and prescriptions that may be made on your behalf.
A valid health card is needed to present to our clinic and see your physician. If your health card is expired, you will need to present to Service Ontario to have it renewed or click on the following link to see about renewing online https://www.ontario.ca/page/get-serviceontario-renewal-reminders?gclid=CjwKCAjwyNSoBhA9EiwA5aYlb0tTS-OdfJgdIO_dv5NUHDJI-a-bl0Hi5uU_CBJCUf8p1duUWhD3GBoCB9cQAvD_BwE&gclsrc=aw.ds
Please note, patients with expired health cards may have to pay out of pocket for the services provided by their health care provider.
Yes you can, we triage and have been booking since the start of the pandemic with the direction of our Physicians to minimize the spread of Covid and help decrease the need for our patients visiting the Emerg Department.
Physician Assistants (PA) are highly skilled health care professionals educated in the medical model who practice medicine.
PAs graduate from a Baccalaureate or Master’s Degree program that may be affiliated with a medical school. PAs practice medicine under the supervision of a licensed physician within a patient-centred health care team.
PAs can work in any clinic setting to extend physician services, complement existing services and aid in improving patient access to health care.
